I, the participant named above, am aware that Alcoholics Anonymous World Services, Inc. (“A.A.W.S.”) intends to produce an audio/visual work described above with the current working title set forth above (the “Work”), and hereby acknowledge that A.A.W.S. is and shall be the owner of all rights, title and interest in and to the Work, including all copyrights, moral rights and other intellectual property rights therein.
In support of A.A.W.S. and its work, I wish to participate in and contribute to the Work with the understanding that A.A.W.S. may use certain methods, including the digital distortion of my voice, blurring or concealment of my face, or other efforts to protect my identity.
Thus, I hereby irrevocably grant to A.A.W.S., its affiliated entities and their representatives, agents and licensees, the right to record, edit and use my name, voice and likeness, in and in connection with the Work, and the advertising, promotion, distribution, exhibition and other exploitation of the Work, including all versions, adaptations, compilations, excerpts, and derivative works thereof, in perpetuity, throughout the world via any medium or device now known or hereafter devised. Further, in the event that I contribute any essays, photographs or other materials to the production, I, hereby grant a non-exclusive, royalty-free, irrevocable and sublicensable license to use such materials in and in connection with the Work in perpetuity, throughout the world via any medium or device now known or hereafter devised.
I understand that and agree that (i) A.A.W.S. has no obligation to use my name, image or likeness or other contribution in or in connection with the Work, (ii) I will not have the right to inspect or approve the final content created, or any use of my name, image, likeness or other contribution as used therein, and (iii) I am not, and will not be, entitled to any fees, royalties or other compensation arising out of the exploitation of any rights granted herein.
I agree that I will not assert or maintain against A.A.W.S., its affiliated entities, their representatives, agents, licensees or any of their successors or assigns (the “Releasees”), any claim, action, suit or demand of any kind or nature whatsoever, including but not limited to those grounded upon violation of rights of privacy or publicity, intellectual property or any other rights, or for any other reason in connection with the use of, or failure to use, my name, voice, likeness and other materials as permitted herein. I hereby release all Releasees from and against any and all claims, liabilities, demands, actions, causes of action(s), costs and expenses whatsoever, at law or in equity, known or unknown, anticipated, which I have now, have ever had, or may, shall or hereafter have by reason, matter, cause or thing arising out of this Appearance Release. I affirm that I have all rights, licenses and releases necessary to grant the rights granted in this Appearance Release, and the exercise of rights granted herein will not violate the rights of any third party.
By agreeing to the terms, I acknowledge and affirm that (i) I am of legal age and otherwise competent to enter into binding contracts (or if not, my legal guardian has executed on my behalf), (ii) I have read this Appearance Release and fully understand the meaning and effect thereof, (iii) I am not entitled to any payment or other compensation, (iv) I am qualified and competent to enter into this agreement based on my age and mental capacity, or if I am not, that my parent or legal guardian has also executed below, and (v) I have signed this Appearance Release intending to be legally bound.
